Types of Information Galen Collects Through its Web Site

Galen collects and stores certain personal information that you may choose to provide. Certain web pages, such as billing and patient scheduling, allow you to input information. Information that you choose to provide could include your name, address, e-mail address, credit card information, or other similar information. Other pages from our web site may be used without any information from you. Personally Identifiable Information

Information that you provide is primarily used for the purposes for which it was collected. For example, any credit card information you provide will be submitted to the credit card processing service provider. It may also be used to respond to your billing questions. Similarly, information you give to request an appointment will be used to schedule your appointment, or notify you of a change.

Notwithstanding anything herein to the contrary, Galen reserves the right to release information collected from a visitor (whether or not personally identifiable), including information contained within our access logs, when that visitor is in violation of our Terms of Use Agreement or is reasonably suspected of any illegal activity, whether or not Galen has been served with a subpoena, warrant or other court order. Changing or Deleting Personal Information

If you would like to have your personally identifiable information removed from Galen’s database, please contact us at privacy@galenmedical.com with your request, and Galen will use reasonable efforts to comply with your request. Keep in mind, however, that residual information always remains within Galen databases, access logs and other records, which may or may not contain such personally identifiable information. Galen reserves the right, from time to time, to re-contact former customers or users of the Galen web site.
